Partners In Care…….
is a membership organization of homeless service
providers, other professionals, units of local and state government, service utilizers, and other community representatives
located in Hawai’i
on Oahu. It is a planning, coordinating,
and advocacy body that develops recommendations for programs and services to fill gaps in the Continuum of Care on O’ahu.
Our Mission…..
Is to solve homelessness through:
Ø Open and inclusive participation, and
Ø Coordinated, integrated, and responsive efforts.
Our Goals….
Are to:
Ø Build and maintain a community-based Continuum of Care process
Ø Advocate for a full continuum of programs and services
Ø Ensure that homeless persons are treated with dignity and care
Ø Engaged in planning and evaluation to maximize the use of existing resources, and
Ø Advocate for policy changes that promote a comprehensive, long-term approach to solve homelessness.

Chairperson, Laura
Thielen
Schedules and facilitates the PIC executive and
general meetings, and any other special meetings or task forces. Serves as the
primary liaison, media contact and/or public speaker on behalf of PIC. Writes
letters on behalf of PIC, attends statewide meetings and other task force meetings related to homeless issues. Serves as the primary contact for police and city officials regarding sweeps, crackdowns, or other activities
related to homeless issues.

Chair, Advocacy, Darryl Vincent
Serves as chair and media contact in the absence
of the chairperson. Schedules and facilitates advocacy meetings and circulates meeting notes.
Develops and advocates for legislative agenda, and presents testimony on behalf of PIC.
Coordinates both local and state advocacy initiatives, and attends related task force meetings.
Vice Chair, Planning , Rona Fukumoto
Serves as primary liaison to state and city homeless
planning divisions and policy academies in relation to homeless issues. Schedules
and facilitates planning meetings and circulates meeting notes. Attends planning
task force meetings on behalf of PIC and participates with long-range planning, funding, and evaluation. Updates the Plan to End Homelessness, provides information to PIC about trends in federal homelessness
funding and informs legislative committee of funding strategies, and maintains status on gaps analysis and the Homeless Management
Information System (HMIS).

Solicits and welcomes new members to PIC. Schedules and facilitates organizational development meetings and circulates meeting
notes. Collects and compiles agency descriptions and updates membership packets. Ensures delivery of memberships packets to new members. Encourages participation by recognizing members for consistent attendance and participation. Contacts and follows up with absentee members.

Coordinates and oversees annual Homeless Awareness
Week activities. Serves as the primary media contact for Homeless Awareness Week. Schedules and facilitates awareness committee meetings and circulates meeting notes.

Oversees and manages finances of PIC, reports
monthly balance at general meetings, makes payments for events and other purchases after approval from PIC membership. Collects dues from members, records receipts, and reports status of members to Organizational
Development Chair.
Secretary, Suzanne Nakano
Serves as general recorder of all PIC history. Records meeting minutes, disseminates information, coordinates meeting dates and locations,
and posts announcements. Maintains and updates PIC roster.

Executive
Facilitated by PIC chairperson and attended by
elected members, city and state representatives, and the previous year’s chairperson.
This committee develops agenda for the PIC general meeting, develops action steps to meet PIC identified priorities,
requests information from the city and state on behalf of PIC, and represents PIC at key city, state, or related governmental
or political meetings.
Advocacy
This committee targets new state legislation and
develops plans of action to ensure passage. Follows local, state, and national
legislation directly related to issues affecting homelessness.
Planning
This committee coordinates with the city on the
annual McKinney-Vento homeless funding application, regularly updates the Plan to End Homelessness, and develops recommendations
for funding priorities and strategies, and other cross-agency funding opportunities.
Organizational Development
This committee works to engage the involvement
of more partners, and schedules member presentations for the PIC general meeting. Develops
membership packets, orientation of new members, and coordinates with Secretary to compile attendance data. Submits assessment of membership participation to the city for annual application evaluation process.
Awareness
This committee coordinates the annual Homeless
Awareness Week and any additional and/or ongoing awareness and education-related activities within the community, political
arena, and the schools.

Services: Aloha United Way is a major fundraiser on behalf of the health and human service providers
on Oahu. AUW is shifting from a focus only on fundraising to a focus that includes community
Issues through investments in long-term solutions to the following community conditions: Homelessness, Crime & Drug Use,
Early Childhood Development and Family & Individual Self-Sufficiency. Finally, we provide “clearinghouse”
services through AUW 211 (24/7 referral hotline), Gifts-in-Kind and Volunteer Hawaii (online volunteer matching system).

Description of
Services: Serves unsheltered homeless
families. The Maili Land Housing Program provides temporary housing and a structured,
programmatic environment to assist families develop the necessary skills, education and income to obtain long-term permanent
housing within 24 months of program intake. General services include: intake,
assessment, referral and case management services, on-site educational classes, employment preparation, guidance in planning
for a successful transition and post-exit follow up services.
Description of
Agency: Catholic Charities Hawaii operates
38 programs, including services provided on Oahu, Big Island,
Kauai and Maui. The agency provides housing
services at two transitional programs: Ma’ili Land (in Waianae) and Kawaihae
(on the Big Island). Catholic Charities Hawaii’s Intake, Information and Referral Program and the
Housing Placement Program also assist individuals and families with housing needs. The
Housing Assistance Program counsels elders (aged 60+) and assists with linkages to affordable housing.

Service Area and Target Population: Homeless families with children in Windward Oahu and Honolulu
Description of
Services: Family Promise of Hawaii provides 24 hour shelter, 3 meals a day, and a
supportive environment for homeless families with children. Guests actively work
with a case manager to accomplish weekly family, housing, and saving goals. Nationally,
80% of families who complete the Family Promise program secure housing and maintain their residence for at least a year. In order to promote a healthy environment for our keiki and volunteers, guests with
current substance abuse or untreated mental illness are not accepted into the program.
Description of
Agency: Family Promise of Hawaii is
a grassroots initiative and collaboration that began locally in the fall of 2004, and was patterned after a successful program
already operating in 38 states. Our mission is to form a community-based `Ohana
to aid families with children experiencing homelessness and help them transition to sustainable independence. Family Promise collaborates with 37 congregations and over 1,000 volunteers to provide overnight accommodations
and meals within their buildings. We provide this support while also connecting
families with community resources for job training, education, life skills, health concerns and housing. This provides a comprehensive approach to helping families that are homeless and directly meets the needs
of the most vulnerable: families with children.
